About The Position

Rehabilitation Hospital of Oak Creek is hiring a Licensed Practical Nurse to provide care in its inpatient rehabilitation facility. The facility is a new, 40-bed hospital offering physical medicine and rehabilitation programs intended to maximize patients’ health, function, and quality of life. The role involves assessing patients, providing and documenting nursing care, educating patients and families, and collaborating with the interdisciplinary team. The position is PRN and works nights.

Requirements

• Active Wisconsin or compact-state LPN license; relocating nurses must update licensure to their new Primary State of Residence before employment begins.
• Associate’s degree required.
• AHA Basic Life Support (BLS) certification required within 30 days of hire.
• Critical-thinking skills and decisive judgment.
• Ability to work with minimal supervision and in a stressful environment, and to take appropriate action.

Nice To Haves

• One year of previous experience preferred.

Responsibilities

• Accurately perform patient assessments and identify patient needs.
• Identify and initiate appropriate nursing interventions.
• Provide patient and family teaching throughout the rehabilitation stay and before discharge.
• Provide care appropriate to the patient’s condition and age.
• Work cooperatively and actively participate with other members of the interdisciplinary team.
• Participate in Continuous Quality and Performance Improvement (CQPI).
• Complete timely and appropriate medical-necessity documentation in the medical record.
• Complete and revise the Interdisciplinary Care Plan for each patient.
• Perform timely and accurate QI assessments.
