About The Position

Requirements

Current LPN license specific to the state or states where assigned to work.
Current CPR certification.
At least 1 year of LPN or LVN experience.

Responsibilities

Verify the plan of care with the RN before each visit.
Collaborate frequently with the physician and RN to ensure patient needs are met, and promptly notify them of changes in a patient's condition.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review medications and perform medication reconciliation at each visit.
Perform specialized procedures ordered by a physician, such as Foley catheter changes.
Prepare equipment and materials for treatments using aseptic technique.
Complete documentation accurately and on time.
