Licensed Practical Nurse All Shifts
About The Position
As a Licensed Practical Nurse (LPN) or Graduate Practical Nurse (GPN) on our Chronic Care team, you'll provide compassionate, patient-centered care to individuals with complex, long-term medical needs. Working under the supervision of a Registered Nurse, you'll collaborate with the interdisciplinary team to assess patient needs, implement individualized care plans, monitor patient progress, and support positive clinical outcomes. Requirements
- Graduate of an accredited licensed practical nurse program
- Current licensure as a licensed practical nurse in the State of Connecticut
Nice To Haves
- Valid/Current CPR certification -required within three months of employment
- Minimum one year LPN experience in a medical/surgical or long-term care setting.
