About The Position

This full-time position for a Licensed Practical Nurse (LPN) in Home Health requires a Monday-Friday schedule from 8:00 a.m. to 5:00 p.m. The primary coverage area includes Calumet City, South Holland, Dolton, Riverdale, Harvey, Dixmoor, Phoenix, Posen, Burnham, Hegewisch, Thornton, Lansing, Glenwood, and Lynwood. The role is eligible for a sign-on bonus of up to $5,000.00, subject to taxes and payout conditions. The LPN works under the direction and supervision of a Registered Nurse (RN), focusing on quality of care, patient safety, care coordination, and patient education within the home health setting.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of Illinois (or eligibility for licensure)
  • Valid driver’s license issued by the Division of Motor Vehicles
  • Current CPR certification
  • Minimum of two (2) years of Med/Surgical clinical experience
  • Compliance with all state continuing education requirements
  • Basic computer skills, including Microsoft Word and email
  • Ability to learn and use electronic scheduling and documentation systems
  • Strong written and verbal communication and documentation skills
  • Effective interpersonal and organizational skills
  • Proficiency in obtaining laboratory draws and reporting results to RN or clinical manager

Responsibilities

  • Facilitates the patient’s and family’s right to receive high-quality, cost-effective care in alignment with Advocate’s MVP of Stewardship, while working under the direction and supervision of a Registered Nurse (RN).
  • Utilizes appropriate resources in response to situations that may negatively impact patient and family outcomes.
  • Adapts practice in accordance with current standards and evidence-based literature.
  • Participates in initiatives to reduce risk and improve patient safety.
  • Demonstrates thorough, accurate, and timely documentation to support reimbursement and regulatory requirements.
  • Functions as an effective member of the patient care team, contributing to an integrated approach to care in alignment with Advocate’s MVP of Partnership.
  • Recognizes changes in patient condition and promptly consults with the RN regarding appropriate actions.
  • Demonstrates sound clinical judgment in caring for patients, including those with complex conditions.
  • Prioritizes and organizes patient care according to the established plan of care.
  • Ensures clear and effective hand-off communication using tools such as SBAR and the 5 P’s.
  • Participates in interdisciplinary case conferences, as appropriate based on employment status.
  • Demonstrates respect and collaboration across disciplines while supporting Advocate’s MVP of Excellence.
  • Participates in staff meetings, agency in-services, and educational activities to support professional development and organizational goals.
  • Utilizes available resources to maintain current knowledge relevant to practice.
  • Demonstrates effective communication, feedback, and conflict resolution skills.
  • Seeks opportunities to enhance personal knowledge, skills, and competencies, and supports departmental development as appropriate.
  • Builds and maintains therapeutic relationships across the continuum of care in alignment with Advocate’s MVP of Compassion.
  • Adapts communication styles to individual patient needs using motivational interviewing and problem-solving techniques.
  • Educates patients and families to safely manage self-care, disease processes, medications, procedures, and infection prevention.
  • Participates in implementing individualized plans of care using SMART goals under RN supervision.
  • Involves patients and families in goal setting and care planning once the RN-established plan of care is in place.
  • Delivers care that routinely exceeds patient and family expectations, as measured by patient satisfaction surveys.
  • Incorporates AHHS Standards of Behavior into all interactions.
  • Demonstrates understanding of healthcare economics and contributes to productivity and efficiency.
  • Adheres to personnel and operational policies.
  • Collaborates with the RN to implement care plans that maximize efficiency and conserve resources.
  • Provides input regarding resource utilization to support optimal patient outcomes, as appropriate.
  • Assesses readiness to learn and adapts educational approaches accordingly.
  • Provides appropriate educational materials using health literacy principles to achieve learning outcomes.
