About The Position

The LPN Charge Nurse supervises, provides, and directs nursing care in compliance with company and facility policies and procedures. This role involves making advanced observations, reporting and recording them appropriately, forming nursing judgments, and carrying out nursing actions based on scientific principles. The position adheres to prescribed policies, procedures, and applicable federal, state, and local regulations, operating under the general supervision of the Director of Nursing.

Requirements

  • Current Nurse License
  • Graduate of an accredited school of nursing or a vocational school of practical nursing
  • Working knowledge of Windows, Microsoft Office Suite and the Internet
  • Good written and verbal communication skills
  • Good communication, comprehension and interpersonal skills
  • Ability to speak, read, write and understand English

  • One to two years of experience preferred

Responsibilities

  • Supervise, provide, and direct nursing care in compliance with facility policies and procedures.
  • Make advanced observations, report and record them appropriately, form nursing judgments, and carry out nursing actions.
  • Follow established standards of nursing practices and assume responsibility for compliance with federal, state, and local regulations.
  • Interpret existing policies to staff, families, and physicians.
  • Perform daily rounds and develop and implement accurate comprehensive care plans based on each resident’s needs.
  • Follow established policies and procedures for passing medications and treatments, and order and credit medications as needed, keeping accurate accounting of all medications.
  • Follow staffing guidelines and make nursing assignments.
  • Receive and give an accurate report on residents’ conditions to provide continuity of care.
  • Monitor diets and supervise meal trays and snacks served to residents, assisting in passing meal trays, feeding, and passing snacks.
  • Follow facility admissions procedures and communicate admissions to other departments, staff, physicians, and other service providers.
  • Ensure employees follow the chain of command consistent with personnel policies.
  • Assign nursing assistants and ensure the efficient completion of their assignments, supervising, documenting, and correcting poor performance.
  • Keep Unit Managers/Director of Nursing informed of poor performance problems.
  • Participate in the performance evaluation process of nursing assistants and provide counseling, coaching, and training.
  • Present performance evaluations to staff.
  • Follow written and verbal directions.
  • Provide exemplary customer service to residents, peers, families, managers, and visitors.
  • Report any changes in physical or mental conditions on a timely basis.
  • Follow all company, facility, and state policies and procedures.
  • Attend staff meetings, in-services, inter-disciplinary team, and committee meetings as assigned.
  • Exercise keen observations of noise, odors, cleanliness, etc., and cooperate with other staff members to ensure quality environmental conditions.
  • Document resident information timely, accurately, and confidentially according to procedures.
  • Document changes and notification to POA and doctor; perform comprehensive assessments as assigned.
  • Verify change-over MARS/TARS are correctly updated and necessary changes are made to new MAR/TAR by the 1st of each month.
  • Adhere to the guidelines as outlined in the Resident Rights documentation.
  • Know resident rights and help residents exercise and/or protect their rights.
  • Report resident complaints to supervisor in charge, maintaining confidentiality.
  • Adhere to all policies and procedures as outlined under the Safety & Compliance policies and procedures.
  • Ensure residents are safe (e.g., Wander guard tab is securely attached to bed/chair as ordered, Wander guard bracelets are working properly).
  • Demonstrate proper use of equipment and report equipment needs or repairs.
  • Use protective equipment, follow infection control protocol standards, policies, and procedures.
  • Practice universal precautions and life safety measures.
  • Demonstrate and maintain a positive, professional manner, manage time efficiently and effectively, provide leadership as a role model to promote a team approach, collaborate and participate in policy creation and appropriate problem-solving methods.
  • Adhere to all policies, procedures, terms, and conditions set forth in the National Church Residences’ Employee Information Guide (EIG), the Code of Conduct, as well as any facility handbook.
  • Ensure all care is provided with respect and dignity for the residents/clients, their families, and the staff.
  • Report all complaints made by residents/clients and/or families to the appropriate supervisors.
  • Report all allegations of abuse, misappropriation of funds/resident property, and/or any other corporate compliance, drug-free workplace, safe work practices, and all federal, state, local regulations and laws.
  • Complete all annual education requirements timely, including but not limited to Compliance, Privacy, Security, and HIPAA training.
  • Exhibit behavior as set forth by the Code of Conduct in the performance of duties.
  • Perform other duties as assigned.
