About The Position
Haven Home Health & Hospice is a locally owned, clinician-led organization that prioritizes quality care over quantity. They are seeking a Licensed Practical Nurse (LPN) to provide skilled nursing services to patients in their homes. The LPN will assess, implement, document, and monitor patient care, working closely with the RN and interdisciplinary team to ensure patients receive consistent, compassionate, and clinically sound care in their homes. Requirements
- Current Missouri LPN license
- At least one (1) year of recent LPN experience (acute, long-term care, or home health preferred)

- Home health experience preferred
Responsibilities
- Provide skilled nursing services to patients in their homes, supporting the established plan of care.
- Assess, implement, document, and monitor patient care.
- Work closely with the RN and interdisciplinary team.
- Recognize and report changes in patient condition to the RN supervisor.
- Complete timely, accurate clinical documentation.
- Support and carry out the patient’s plan of care alongside the RN.
- Collaborate with therapists and other clinicians to ensure continuity of care.
