About The Position
Mountain Valley Hospice offers compassionate, personalized care to patients and their families during life’s most challenging moments. They are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join their team of professionals, providing exceptional care in patients’ homes, skilled nursing facilities (SNFs), and assisted living facilities (ALFs). As a part of the team, the LPN will collaborate with nurses, social workers, chaplains, and providers who share a deep commitment to delivering high-quality care and support. The role involves making a meaningful difference in the lives of those served.
Requirements
- Valid Licensed Practical Nurse License
- Exceptional interpersonal and communication skills, with a compassionate and patient-centered approach.
- Ability to work both independently and as part of a team, with excellent time management skills.
Responsibilities
- Provide direct care to patients, including administration of medication, wound care, treatments, and procedures under the supervision of a registered nurse.
- Work with the interdisciplinary team to address patient and family needs, including connecting with other providers (aides, social workers, chaplains, and more).
- Recognize and document changes in patient conditions and communicate these effectively to the care team.
