About The Position

Journey is seeking a Licensed Practical Nurse to support resident care and direct daily nursing activities. The role follows the nursing model to promote holistic care and helps ensure nursing personnel comply with applicable regulations, guidelines, and facility policies and procedures. The nurse participates in resident admissions, discharges, and transfers; monitors residents for changes in status; notifies physicians and residents’ families or representatives as appropriate; documents changes; and transcribes and carries out physician orders.

Requirements

Nursing degree from an accredited college or university.
Current unrestricted license as a Registered Nurse (RN) in the practicing state.
Current CPR certification.

Nice To Haves

Prior experience in long-term care is preferred but not required.

Responsibilities

Direct the daily activities of licensed practical/vocational nurses and certified nursing assistants in accordance with applicable regulations, guidelines, and facility policies and procedures.
Promote holistic resident care by following the nursing model.
Ensure assigned nursing personnel comply with policies and procedures.
Participate in resident admissions, discharges, and transfers as required.
Assess residents for changes in status, notify the physician and the resident’s family or representative, and document accordingly.
Transcribe physician orders to the medical record and carry them out as written.
