About The Position

Our unique Cape Code style Long Term Care Houses offer LPNs excellent resident to nurse ratios of 1:12 on days and evenings and 1:24 for nightshifts! The Licensed Practical Nurse (LPN) is responsible for the overall functioning of the assigned house during their shift, including providing supervisory direction to Nurse Aides. This role involves establishing work assignments for unit Nurse Aides and supervising them to ensure thorough task completion. The LPN will report to the acting Supervisor on data that needs coordination with other disciplines. Responsibilities include the timely and accurate administration and documentation of physician-ordered medications and/or treatments, maintaining facility procedures for ordering and receiving medications, and monitoring residents for changes in condition with appropriate chart documentation. The LPN will refer problems to the acting Supervisor, document in electronic medical records according to facility policy, and provide shift input on resident care needs for the Interdisciplinary Care Plan. Maintaining awareness of changing resident needs, both physical and emotional, and adjusting work assignments and resident priorities to maintain accepted care standards are crucial. The LPN will maintain documentation of shift occurrences for thorough reporting to the oncoming shift, report changes in residents' condition to the Charge Nurse or Supervisor, and have access to the entire medical record of all patients/residents. Consultation with the disciplinary team for information, notification to families, physicians, and/or administration is also part of the role. The LPN will be proactive in the preventative/protective skin care program and perform duties according to the highest ethical and moral standards, adhering to the facility's corporate compliance plan and applicable governing laws. Adherence to the facility's confidentiality statement regarding protected health information (PHI) is required. The LPN will perform any other duties assigned by the shift supervisor and assure completion of monthly reports as requested by Nursing Administration.

Requirements

  • Graduate of an accredited School of Practical Nursing.
  • Current Licensure as a Licensed Practical Nurse in New York State.
  • Leadership ability and effective communication skills.
  • Demonstrates competency by completing annual job specific skills of unit.

Responsibilities

  • Provides shift input of resident care needs to the Interdisciplinary Care Plan for continuity of resident care.
  • Maintains awareness of changing resident needs, both physical and emotional.
  • Adjusts work assignments, resident priorities etc. to assure accepted care standards are maintained.
  • Maintains documentation of shift occurrences to promote thorough repotting of resident activity to oncoming shift.
  • Reports changes in residents' condition to Charge Nurse or Supervisor.
  • Access to entire medical record of all patients/residents.
