About The Position

Galileo is a team-based medical practice focused on improving healthcare quality and affordability. We provide high-touch, data-driven, multi-specialty, longitudinal care to diverse and complex patients across 50 states, utilizing phone, home visits, and other means. Our mission is to transform society by solving complex healthcare problems and bringing patients and providers closer together. The Clinical Health Advocate (Licensed Practical Nurse) is a crucial part of our care team, responsible for patient relationships and collaborative care plans. This is a part-time (up to 24 hours/week) hybrid role involving field visits, office work in Presque Isle, and some remote work. The role is 50% clinical and 50% administrative, with general work hours Monday through Friday from 8 am to 5 pm ET.

Requirements

  • At least 2 years clinical experience as Licensed Practical Nurse
  • Experience with phlebotomy is a must

Responsibilities

  • Obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR during patient visits.
  • Perform venipuncture and capillary blood collection for laboratory testing following proper phlebotomy protocols.
  • Collect, process, and ready any specimens for laboratory analysis.
  • Administer immunizations and injections (as able) per physician orders and maintain accurate vaccine records.
  • Assist providers with minor procedures and examinations as needed.
  • Relay pertinent environmental advantages/disadvantages that will impact patient well-being to the care team when in the field, completing physical and social screenings as assigned by the provider.
  • Educate patients on treatment plans, medications, and follow-up care as directed by providers.
  • Document patient interactions, clinical data, and care activities accurately in the electronic health record.
  • Carry out Pop Health care gap closures efforts and sprints as needed per payer contract.
  • Ensure proper sterilization and maintenance of medical equipment.
  • Follow infection control protocols and maintain compliance with safety standards.
  • Be available for field and urgent deployment within your market region even on unplanned field days.
  • Connect patients with various Galileo services and teams, including clinical and administrative functions.
  • Represent the organization professionally, embodying its mission, and creating meaningful, lasting relationships with all stakeholders.
  • Own overall empaneled patient chart management, prepping charts medically and demographically to ensure readiness and thoroughness of care.
  • Run-The-List in partnership as a care team to ensure all orders, care coordination, and constant communication is seamless across each empaneled patient's care plan.
  • Maintain exam room, mobile unit, and/or office cleanliness, organization, and adequate supply levels.
  • Manage inventory of clinical supplies and medications, placing orders as needed.
  • Route inbound mail appropriately, sending patient letters or medical equipment as needed.
  • Coordinate with insurances to submit and complete prior authorizations and referrals.
  • Request, follow up, and file medical records with local hospital systems, PCPs, and area HIEs.
  • Submit Pop Health documents to maximize compliance with payer contracts as needed.
  • Communicate with patients via text, video, or phone as needed for care coordination, scheduling, and reactive needs.
  • Participate in projects and events as assigned by the Director of Operations.
