Requirements

  • Associate’s Degree in Nursing or related field, or state certificate
  • One year of experience working with population served in a nursing capacity
  • Current LPN licensure for state of residence
  • Current driver's license in good standing
  • Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
  • Ability to effectively communicate with staff, individuals served, and families

Responsibilities

  • Monitor the health conditions of individuals served and ensure quality services are provided.
  • Administer medications and provide documentation as well as oversee and monitor staff members who administer medications.
  • Completes health assessments and provides input of health diagnosis to RN or physician for the development of nursing care plan.
  • Follows-up on all physician orders and monitors and ensures follow through on individual’s medical/dental appointments.
  • Monitors and records medications in a timely manner; acts as a liaison to pharmacy.
  • Reviews medication error forms, analyzes errors and makes recommendations for procedural changes.
  • Administers medication as required.
  • Oversees and monitors staffs who administer medication.
  • Conducts quality assurance checks on health care records.
  • Provides emergency coverage and/or care for individuals receiving services.
  • Maintains confidentiality, respects human rights and practices universal precautions in accordance with Mentor policy.
  • Upholds policy and procedure development related to health care.
  • Works with external agencies to coordinate services and ensure consistent treatment of clients.
  • Administers, tracks, and maintains log for staff and individual’s testing.
  • Provide prescribed medical treatment by physician’s orders and personal care services.
  • Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition.
  • Provide training to staff members including universal precautions, infection control, and medication administration.
  • Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care.
  • Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.
  • Performs other related duties and activities as required.
  • May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities, appointments.
