Requirements

Graduate of an approved Practical Nursing or Vocational Nursing program.
Current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
Current CPR/BLS certification through the American Heart Association.
Strong knowledge of nursing practices, medication administration, and resident care standards.
Ability to effectively communicate with residents, families, physicians, and interdisciplinary team members.
Knowledge of state and federal regulations governing long-term care and skilled nursing facilities.
Experience with electronic health records (EHR) and clinical documentation systems.
Must undergo a post-offer drug test and federal background investigation.

Nice To Haves

One (1) year of LPN experience preferred, preferably in a skilled nursing, long-term care, rehabilitation, or healthcare setting.
Previous experience in a skilled nursing, long-term care, rehabilitation, hospital, or assisted living setting.

Responsibilities

Follow assigned duties provided by the charge nurse.
Provide appropriate nursing interventions for patient care and safety.
Administer medications according to prescribed orders in a timely manner.
Document medications and procedures in the medical record.
Work as a team member to facilitate patient admission and discharge processes.
Transcribe orders accurately and audit the medical record for accuracy.
Act as a patient advocate, providing education to patients and families.
Communicate with the nursing team to fulfill patient needs.
Assist with activities of daily living and encourage patient independence.
Communicate with medical staff regarding identified patient issues.
Perform patient care assessments timely and accurately.
Address patient care needs promptly and document findings accurately.
Ensure daily documentation is completed thoroughly, signed, and dated according to protocol.
Adhere to Human Resource policies regarding code of conduct and attendance.
Follow Infection Control and Employee Health Guidelines.
Work with the Service Development Department to facilitate timely admissions.
Function as a member of the multidisciplinary team, communicating patient goals, interventions, and assessments.
