Requirements

  • Graduate of an approved Practical Nursing or Vocational Nursing program.
  • Current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
  • Current CPR/BLS certification through the American Heart Association.
  • Strong knowledge of nursing practices, medication administration, and resident care standards.
  • Ability to effectively communicate with residents, families, physicians, and interdisciplinary team members.
  • Knowledge of state and federal regulations governing long-term care and skilled nursing facilities.
  • Experience with electronic health records (EHR) and clinical documentation systems.
  • Must undergo a post-offer drug test and federal background investigation.

Nice To Haves

  • One (1) year of LPN experience preferred, preferably in a skilled nursing, long-term care, rehabilitation, or healthcare setting.
  • Previous experience in a skilled nursing, long-term care, rehabilitation, hospital, or assisted living setting.

Responsibilities

  • Follow assigned duties provided by the charge nurse.
  • Provide appropriate nursing interventions for patient care and safety.
  • Administer medications according to prescribed orders in a timely manner.
  • Document medications and procedures in the medical record.
  • Work as a team member to facilitate patient admission and discharge processes.
  • Transcribe orders accurately and audit the medical record for accuracy.
  • Act as a patient advocate, providing education to patients and families.
  • Communicate with the nursing team to fulfill patient needs.
  • Assist with activities of daily living and encourage patient independence.
  • Communicate with medical staff regarding identified patient issues.
  • Perform patient care assessments timely and accurately.
  • Address patient care needs promptly and document findings accurately.
  • Ensure daily documentation is completed thoroughly, signed, and dated according to protocol.
  • Adhere to Human Resource policies regarding code of conduct and attendance.
  • Follow Infection Control and Employee Health Guidelines.
  • Work with the Service Development Department to facilitate timely admissions.
  • Function as a member of the multidisciplinary team, communicating patient goals, interventions, and assessments.
