About The Position

UCP Long Island is a not-for-profit agency dedicated to advancing the independence, productivity, and full citizenship of individuals with cerebral palsy and other disabilities. With over 75 years of service, we create opportunities and help fulfill dreams. We are seeking a dedicated Licensed Practical Nurse for a part-time, 32-hour per week position within our Residential program in Central Islip. This role involves assisting in the overall health and safety of the individuals served, under the supervision of a Registered Nurse. The position offers flexible hours based on the needs of the individuals.

Requirements

  • Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.
  • Licensed Practical Nurse (LPN) license.

Responsibilities

  • Knowledge of all medications for all individuals.
  • Administer medications and treatments as prescribed by physician.
  • Generate incident, seizure, and illness reports.
  • Monitor and report health care on daily flow report records.
  • Instruct mid-level supervisors and direct care staff in hygiene, diet, weights, etc.
  • Submit written monthly, quarterly, annual nursing/medical progress notes.
  • Coordinate laboratory care and medical appointments (routine, fees, special and emergency).
  • Maintain functioning interpersonal relationships with other disciplines.
  • Maintain ongoing communication with other programs, i.e., client health care.
  • Monitor program participants during mealtimes.
  • Order, record, and maintain sufficient/adequate supply of medication.
  • Maintain sufficient/adequate amount of medical supplies/materials.
  • Maintain accurate medical records including medication administration sheets, Kardex, Formulary, "Pick-up" and follow through on all physician orders.
  • Provide emergency first aid.
  • Coordinate emergency services.
  • Supervise direct care staff in absence of mid-level supervisor.
