About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role involves collaborating with physicians and the RN, supporting patients and caregivers, and completing patient-care documentation accurately and on time.

The position has a stated annual total cash target pay range of $49,151–$73,726, a per-visit-point rate of $28.36–$42.53, and an hourly rate of $23.63–$35.45. The annual total cash estimate assumes full-time employment at 40 hours per week and full productivity. Compensation varies based on factors including local labor markets, education, work experience, productivity, and performance. Visit work is paid using a per-visit-point rate; certain non-visit activities, such as orientation, are paid hourly. Requirements

• Current LPN license specific to the state or states assigned for work.
• Current CPR certification.
• At least 1 year of LPN or LVN experience.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Verify the plan of care with the RN before each visit.
• Collaborate frequently with the physician and RN to ensure patient needs are met, and notify them as soon as possible when a patient's condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by a physician, such as Foley catheter changes.
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation accurately and promptly.
