Licensed Practical Nurse (LPN) – Per Diem Home Health –South Broward County
About The Position
Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses to perform home health visits in South Broward County. The role involves initiating patient care, completing comprehensive assessments, and maintaining accurate OASIS documentation for patients receiving home-based care, often after discharge from a hospital, rehabilitation center, or nursing facility. The nurse delivers skilled nursing services, manages assigned cases, and coordinates care with patients, families, physicians, and interdisciplinary teams. The position also involves supporting and educating patients and family members, collaborating with other care providers, monitoring outcomes, and coordinating discharge from services.
Requirements
- Active Florida Licensed Practical Nurse (LPN) license in good standing.
- At least 1 year of home health experience.
- Strong experience with OASIS documentation.
- Excellent clinical assessment and critical thinking skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and a valid driver’s license.
- Subject to background screening under Chapter 435, Florida Statutes, including fingerprinting and eligibility review before employment.

Responsibilities
- Provide home-based care to patients with illnesses, injuries, and chronic conditions.
- Examine patients, complete clinical assessments, document progress, and report changes to physicians.
- Complete timely clinical documentation, including OASIS, evaluations, physician orders, and care coordination notes.
- Develop, implement, and monitor patient care plans in collaboration with patients and their representatives.
- Deliver skilled nursing services and provide case management for assigned patients.
- Educate patients about their plan of care, medications, therapies, diet, and exercise.
- Coach and support family members on patient care, medication management, and home safety.
- Coordinate care with physicians, discharge planners, and interdisciplinary teams.
- Oversee and collaborate with LPNs, CNAs, and home health aides providing patient care.
- Monitor patient progress, outcomes, and changing needs, and adjust care plans accordingly.
- Ensure care goals are met and coordinate appropriate discharge from services.
- Liaise with community health resources, including DME providers, oxygen services, and infusion therapy providers.
- Maintain ongoing communication with patients, families, physicians, clinical managers, and office staff.
- Participate in interdisciplinary team meetings and staff conferences.
- Engage in continuous education and professional development opportunities.
- Develop and follow work methods and procedures that support high-quality patient care.
