About The Position

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team and contribute their talent and passion while providing Joyful Service. This full-time day shift position offers a hiring bonus of $1,500, with the first half paid after 90 days and the second half after six months of employment. The salary range for this role is $25-$29 per hour, with pay determined by verifiable experience. The LPN will be responsible for coordinating with the healthcare team to deliver quality care and restorative services, aiming to maximize resident health potential, functional capacity, and quality of life. This includes providing direct and indirect nursing care, resident assessment, care planning, intervention, and evaluation. The LPN will also direct and supervise nursing assistants, ensuring the provision of quality care and health services in compliance with professional nursing standards and regulations.

Requirements

  • Successful completion and graduation from an accredited school of Licensed Practical Nursing
  • Current, unencircable license to practice Licensed Practical Nursing in Louisiana
  • Current certification for Health Care Provider CPR
  • At least one year of experience
  • Outstanding customer service skills
  • Ability to maintain confidentiality
  • Good communication skills
  • Ability to relate well with seniors and their family members
  • Ability to understand written and verbal instructions and relay information to others
  • Good attendance and punctuality

Responsibilities

  • Coordinate with the health care team to deliver quality care and restorative services to maximize resident health potential, functional capacity, and quality of life.
  • Provide direct and indirect nursing care, resident assessment, care planning, intervention, and evaluation to meet resident needs.
  • Direct and supervise nursing assistants and ensure the provision of quality care and health services in compliance with professional nursing standards of practice and state and federal regulations.
  • Demonstrate leadership, problem-solve using effective management techniques, and delegate tasks appropriately.
  • Supervise and coordinate care/service delivery by nursing assistants, restorative nursing assistants, and ancillary services.
  • Serve as charge nurse in the absence of an RN.
  • Communicate resident information effectively to other members of the interdisciplinary team.
  • Ensure resident safety, security, and rights at all times.
  • Coordinate resident admissions, transfers, and discharges.
  • Initiate assessment, care plan, intervention/treatment, and outcomes measurement.
  • Consult with attending physicians to provide quality resident care.
  • Communicate resident status accurately and facilitate physician examination and treatment.
  • Obtain vital signs and perform blood sugar tests as ordered by the physician and report any abnormalities.
  • Requisition and facilitate diagnostic and therapeutic services as ordered by the physician.
  • Review resident charts for accuracy and completion.
  • Communicate and collaborate with interdisciplinary team members to promote quality resident care and services.
  • Deliver professional nursing services such as assessment, treatment & outcome evaluation, medication administration, parenteral and enteral therapy, airway maintenance, wound care, and care of the dying resident.
  • Include family and significant others in resident assessment, care planning, and treatment when appropriate.
  • Clean and store equipment.
  • Administer medications, including oral, topical, and injectable, as ordered by the physician.
  • Review medication records for completion and accuracy.
  • Ensure narcotic records are accurate for each shift and notify the Director of Nursing of discrepancies.
  • Dispose of drugs as required and in accordance with policy.
  • Promote adequate resident nutrition.
  • Assist in resident dining tasks, including escorting residents, delivering trays, and assisting with meal serving and feeding.
  • Complete accurate, effective Nursing Documentation.
  • Complete and file forms/charts from admission through discharge in accordance with regulations and procedures.
  • Receive and accurately document verbal/telephone physicians’ orders.
  • Complete Nurses’ Notes in an informative and descriptive manner.
  • Document all resident accidents/incidents in accordance with policy.
  • Communicate changes or new orders to appropriate interdisciplinary team members.
  • Report all discrepancies noted in physician orders, charting, or medication/treatment administration to the Director of Nursing.
  • Perform all routine documentation duties as required, including the Behavioral Monitoring Form.
  • Complete and investigate incident reports timely.
