About The Position

Angels of Care Pediatric Home Health is hiring Licensed Practical Nurses to provide one-on-one, in-home care for medically fragile children and young adults in Columbia, South Carolina, and surrounding areas. The role focuses on building meaningful relationships with patients and families while providing high-quality, individualized care in a home health setting. Pay starts at $29 per hour. Full-time, part-time, and PRN opportunities are available, with flexible scheduling and 8-, 10-, 12-, or 16-hour shifts. Day, night, and weekend shifts are available.

Requirements

• Active, unrestricted Licensed Practical Nurse (LPN) license in South Carolina.
• License must have been acquired at least one year before application.
• Ability to provide care in a home setting.
• Strong communication and clinical decision-making skills.

Nice To Haves

• Pediatric or home health experience.
• Experience with high-acuity skills such as tracheostomy, ventilator, or G-tube care.

Responsibilities

• Provide skilled nursing care in a home health setting.
• Perform ongoing patient assessments and clinical evaluations.
• Administer prescribed medications, treatments, and therapies.
• Deliver care in accordance with physician orders and the plan of care.
• Coordinate care with families, providers, and care teams.
• Educate patients and family members to support improved outcomes.
• Implement preventive measures to ensure patient safety and quality care.
• Respond appropriately to changes in a patient’s condition.
