About The Position

BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hudson County Visits team, providing one-on-one skilled nursing visits to adult clients throughout Union County, NJ. As a BAYADA home health nurse, you'll have the opportunity to build meaningful relationships with your clients while providing high-quality care in the comfort of their homes. You'll work independently while being supported by an experienced interdisciplinary clinical team dedicated to helping clients remain safe and independent at home.

Requirements

Current New Jersey Licensed Practical Nurse (LPN) license.
Minimum of one year of recent, verifiable clinical experience as an LPN.
Graduate of an accredited and approved nursing program.
Ability to work independently and effectively manage time.
Strong communication, interpersonal, and organizational skills.
Reliable transportation and ability to travel to client homes throughout Union County.

Nice To Haves

Home health experience strongly preferred, but not required.
Experience with electronic medical records (EMR) preferred.

Responsibilities

Provide skilled nursing visits to clients throughout Union County.
Administer medications, provide wound care, and perform physician-ordered treatments and procedures.
Monitor client conditions and communicate changes to the Clinical Manager and Client Services Manager.
Complete nursing assessments as appropriate.
Follow, execute, and accurately document physician orders.
Coordinate care with physicians and members of the interdisciplinary care team.
Educate clients and caregivers to support health, safety, and independence at home.
Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.
