About The Position

The Licensed Practical Nurse (LPN) provides direct clinical support in the delivery of preventive and primary care services within the HSVS 29-i Medical Clinic. Working under the supervision of the RN Supervisor and in collaboration with the Medical Director, Physician(s), and Nurse Practitioner(s), the LPN supports patient care, ensures accurate documentation, and assists in maintaining compliance with New York State Department of Health (NYSDOH) 29-i Health Facility requirements. The LPN plays a key role in patient preparation, immunization support, clinical procedures, and ensuring the overall efficiency of clinic operations.

Requirements

  • Active Licensed Practical Nurse (LPN) license in New York State required
  • Minimum 1–3 years of clinical experience, preferably in outpatient, pediatric, or adolescent care
  • Experience with Electronic Health Records
  • Strong clinical and organizational skills
  • Attention to detail and documentation accuracy
  • Ability to work in a fast-paced clinical environment
  • Effective communication and teamwork skills
  • Understanding of patient confidentiality and compliance requirements

  • Knowledge of immunization practices and preventive care preferred

Responsibilities

  • Provide nursing care within scope of practice under the supervision of the RN Supervisor
  • Obtain and document vital signs, medical histories, and patient information
  • Prepare patients for examinations and assist providers during visits
  • Assist with clinical procedures and point-of-care testing as directed by licensed providers
  • Perform capillary blood collection (fingerstick) for point-of-care testing, including hemoglobin and lead screening
  • Perform venipuncture blood collection on pediatric and adolescent patients
  • Observe and report patient conditions, concerns, or changes to RN Supervisor or provider
  • Administer immunizations as ordered by a licensed provider and in accordance with NYS guidelines (as permitted within LPN scope)
  • Ensure accurate documentation of immunizations in the EHR and applicable registries
  • Support preventive care services, screenings, and follow-ups required under 29-i standards
  • Accurately document all nursing care and patient interactions in the Electronic Health Record
  • Assist providers in ensuring medical records are complete, timely, and compliant
  • Ensure documentation supports billing requirements and clean claim submission
  • Maintain confidentiality and compliance with HIPAA regulations
  • Support efficient patient flow and clinic operations
  • Prepare and maintain exam rooms and clinical areas
  • Ensure necessary equipment and supplies are available for patient care
  • Implement clinician directives accurately and promptly
  • Assist with specimen collection, handling, and preparation (within scope)
  • Coordinate with Medical Assistants and external laboratories for specimen processing
  • Ensure lab results are received, documented, and communicated to providers
  • Ensure proper use, cleaning, and maintenance of medical equipment
  • Follow infection control protocols and clinic safety procedures
  • Report any equipment issues to RN Supervisor
  • Support readiness for annual equipment inspections
  • Participate in Quality Assurance/Performance Improvement (QAPI) activities
  • Assist with chart reviews by ensuring documentation completeness
  • Maintain compliance with NYSDOH 29-i standards and internal clinic policies
  • Support audit readiness activities
  • Collaborate with RN Supervisor, Medical Assistants, and providers to ensure coordinated care
  • Communicate effectively regarding patient care needs and clinic operations
  • Support training and guidance of Medical Assistants as appropriate
  • Provide patient-centered care that is respectful of cultural, social, and linguistic needs
  • Promote a welcoming and supportive clinical environment
  • Participate in clinic planning and process improvement initiatives
  • Assist in reviewing and updating clinic policies and procedures
  • Perform other duties as assigned by RN Supervisor or clinic leadership
