About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one private duty nursing care for a 2-year-old male child with cardiac needs in a home setting. This role emphasizes quality, personalized care and building meaningful connections with the child and family. The position offers a supportive environment for nurses who wish to focus on patient care and see the direct impact of their work.

Requirements

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care

Nice To Haves

• Home health experience preferred

Responsibilities

• Assist with Activities of Daily Living (ADLs)
• Perform g/gj-tube care/feedings/aspiration precautions
• Monitor vitals
• Administer medications safely and accurately
• Communicate clearly and compassionately with the care team and family
• Deliver exceptional, family-centered pediatric care
