About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one private duty nursing care for a 16-year-old male child with trach/vent needs. This role offers the opportunity to build meaningful connections, deliver personalized care in a supportive home setting, and make a significant impact on a child's life. The position focuses on quality care and fostering a trusted relationship with the child and family.

Requirements

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care

Nice To Haves

• Home health experience preferred

Responsibilities

• Assist with Activities of Daily Living (ADLs)
• Perform g-tube care/feedings/aspiration precautions
• Monitor vitals
• Monitor seizures
• Trach/vent care/monitoring
• Administer oxygen
• Administer medications safely and accurately
• Communicate clearly and compassionately with the care team and family
• Deliver exceptional, family-centered pediatric care
