About The Position

The Licensed Practical Nurse (LPN) is continuously responsible for providing professional nursing services, according to qualifications and training, and non-professional services as directed. The LPN is directly responsible for performing patient care functions according to established standards, policies, and procedures. This role is integral to the community, aiming to provide the highest quality healthcare and customer satisfaction with compassionate, capable, and personalized treatment.

Requirements

  • Graduate of a course for LPN.
  • Must be licensed in the state of Louisiana.
  • CPR certified.
  • No experience necessary.
  • Excellent communication skills (oral and written)
  • Familiarity with electronic medical records (EMR)
  • Interpersonal skills and compassion
  • Analyzing information, critical thinking, and problem solving
  • Attention to detail to ensure accuracy
  • Basic computer skills
  • Remain in a stationary position for extended periods of time.
  • Occasionally move about the office and facility to access various departments and patient care areas.
  • Constantly position self to maintain equipment and provide patient care.
  • Constantly operates a computer, office, and patient care machinery.
  • Ability to communicate information accurately so others will understand.
  • Ability to observe details at close range.
  • Occasionally assist patients such as lifting, pushing, or pulling, up to 50 lbs.
  • Frequently move boxes, equipment, etc. weighing up to 20 pounds across the office for various needs.

Responsibilities

  • Obtain and record vital signs, assessments, chief complaints, medical history, height/weight, etc. as appropriate
  • Perform basic clinical procedures such as wound dressing, specimen collection, point of care testing, EKGs, etc. (as trained)
  • Administer and record all medications and treatments based on provider orders accurately using proper procedures
  • Record all care information concisely, accurately, and completely in a timely manner
  • Assist providers with exams, minor procedures, and urgent care needs
  • Prepare, stock, and sterilize exam rooms and clinical equipment
  • Communicate test instructions and follow-up plans to patients clearly and effectively
  • Assist with patient outreach for lab results, chronic disease follow-up, and preventative-care reminders
  • Collaborate with the care team to ensure timely documentation and quality care delivery
  • Accurately document all patient interactions, procedures, medications, and communications in the Electronic Health Record
  • Follow RHC regulations, safety protocols, and infection control standards
  • Monitor and maintain clinic supplies and equipment usage. Notifies appropriate departments to replenish stock, as necessary.
  • Participate in Performance Improvement on a monthly basis.
  • Maintain knowledge regarding use of defibrillators, holter monitors, etc.
  • Perform and documents quality controls daily
  • Uphold HIPAA and patient privacy standards
  • Provide basic health education, medication instructions, and lifestyle guidelines within skill level and training
  • Maintains a neat and orderly work area.
  • Assists with diabetic foot care, as needed
  • Performs other duties as assigned.
