About The Position

Pinnacle Home Care is hiring a Licensed Practical Nurse to provide compassionate, patient-centered home health care in Deland and surrounding communities. The role delivers individualized care in patients’ homes and administers skilled nursing care under RN supervision, following patient care plans and agency policies. The position requires home visits and completion of accurate, timely documentation in the electronic medical record.

Requirements

• Current LPN or LVN license appropriate to the state.
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least one year of nursing experience.
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health or medical/surgical nursing experience is preferred.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks outlined in patient care plans.
• Use clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
• Complete visit reports promptly and accurately in the electronic medical record, including all required documentation.
• Confirm daily visit schedules to coordinate care with other staff.
• Understand the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with agency service agreements and admission policies.
• Understand agency policies on appropriate patient discharge and reasons for discharge.
