About The Position
Luxe Home Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. This is a per diem position offering flexibility and the opportunity to make a difference in the lives of our patients. As an LPN at Luxe Home Health, you will play an essential role in providing high-quality care to patients in their homes, working under the direction of a registered nurse. You will work with a supportive team to ensure patients' needs are met while maintaining the highest standards of care.
Requirements
- Current, valid LPN license in the state of Indiana.
- Minimum of 2 years of experience as an LPN in a home health setting.
- Ability to work independently and manage time efficiently.
- Compassionate and patient-centered approach to care.
- Valid IN driver’s license, reliable transportation, and the ability to travel within the service area.
Responsibilities
- Administering medications and treatments as prescribed by physicians
- Monitoring and assessing patients' conditions while reporting any changes to the clinical team
- Providing education to patients and their families regarding care plans and health management
- Collaborating with other healthcare professionals to ensure comprehensive care
- Maintaining accurate patient records and documentation according to policies and regulations
