Geisinger 65 Forward is a new kind of primary care, designed just for people 65 and older. This role provides primary clinical support to the provider, patient, and their family to assist in the diagnosis and treatment of the patient's condition. The LPN will follow up on diagnostic testing to ensure completion, accuracy, and filing in the medical record for continuity and quality of care. They will also promote a clean environment and meet the qualifications to provide care for patients in specific age ranges on their assigned clinical area. The LPN will obtain and document patient medical history, chief complaint, vital signs, and provide basic medical data for the provider, preparing the patient for examinations. They will provide direct clinical care as directed by the provider, assist the provider in minor surgical diagnostic procedures and ancillary diagnostic examinations, and collect and prepare laboratory specimens with associated paperwork. The LPN will execute and document ongoing patient communication, prioritizing communication to the provider, and respond to patient questions and requests. They will administer medications and vaccines, including intradermal and skin testing, under the direction of a provider and per scope of practice. Additionally, the LPN will clean and set up examination and treatment rooms between cases, ensuring asepsis, availability of instruments and equipment, and patient preparation. Work is typically performed in a clinical environment. The LPN is accountable for satisfying all job-specific obligations and complying with all organization policies and procedures.
