We are looking for compassionate Licensed Practical Nurses (LPNs). This integral position carries out all nursing care, treatment, and services in a professional and prescribed manner within scope of license.

Requirements

  • Current or temporary license to practice as a Licensed Practical Nurse in the State of Arizona
  • Current Negative TB skin test (Mantoux skin test) within the last twelve months or a written statement from a Physician, Physician’s Assistant or a Registered Nurse Practitioner indicating freedom from Tuberculosis, if in the past has had a positive skin test or Tuberculosis
  • Must be able to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety
  • Current CPR card that was obtained by physical assessment
  • Knowledge of gerontological/geriatric nursing theory and practice
  • Knowledge of resident condition and changing medical/physical /psychological care needs
  • Knowledge of effects of the aging process
  • Knowledge of practices and procedures of team nursing
  • Knowledge of practices and procedures for implementing treatment plans for resident/patient care
  • Knowledge of practices and procedures of pharmacology and medicines
  • Knowledge of Federal, State and agency laws, rules and regulations governing general practical nursing practices and the security and control of medical supplies and medications and dispensing thereof.
  • Skill in practical nursing methods and techniques
  • Skill in observing and reporting behavior and progress of residents/patients
  • Skill in applying and interpreting State and agency laws and regulations governing geriatric nursing practices
  • Skill in assisting in the development and implementation of patient care plans within a team nursing concept
  • Skill in oral and written communication
  • Skill in interpersonal communication
  • Ability to assess client needs and evaluate usefulness or suitability of resources
  • Ability to establish and maintain excellent working relationships with clients, client families, team members and service providers

  • One or more years of experience in a skilled nursing/ long-term care facility

Responsibilities

  • Administer medication and performs treatments on an assigned unit as ordered by the physician and within scope of license (Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care and catheter care.)
  • Observe and report the effectiveness of medications and treatments
  • Ensure an adequate amount of supplies and medication are available
  • Receive and deliver a variety of information regarding resident medical, social, functional, and other needs
  • Participate in receiving and giving of shift reports
  • Communicate with residents, family, and staff as necessary to ensure residents receive appropriate care from the interdisciplinary team
  • Complete the initial admission nursing assessment tool on a newly admitted resident
  • Assist with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews, and care plans
  • Supervise subordinate staff and assist with direct resident care for the completion of Activities of Daily Living (ADL’s) including bathing, dressing, observations, treatment, and summary of resident progress
  • Conduct and participate in in-service training and staff development programs for regularly employed staff and other duties as assigned
  • Train/orient new employees
  • Perform technical duties as assigned (e.g., collect specimens, respond to emergency situations rapidly and appropriately)
