About The Position

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) for a 12-week contract to deliver high-quality care to patients. This role involves providing direct patient care primarily in a clinic setting under the direction of the Registered Nurse and team. The LPN will assist with clinical care coordination, monitor patient conditions, and support the delivery of patient-centered care while maintaining the highest standards of nursing practice.

Requirements

  • Graduate of an accredited Practical Nursing program required.
  • Current LPN License required - South Carolina License/Compact License in good standing.
  • Minimum of 1 year of nursing experience required.
  • Must provide a valid driver's license and proof of auto insurance.
  • Current CPR/BLS certification required.
  • Ability to pass drug screening and background checks.
  • Up-to-date required immunizations (Tdap, MMR, Varicella, Flu) and physical exam or statement of good health.
  • Must meet all applicable health and physical requirements necessary to safely provide patient care.

Nice To Haves

  • home health, long-term care, geriatrics, or managed care experience preferred.
  • Experience working with a frail or elderly population preferred.
  • Strong clinical assessment, communication, and documentation skills.

Responsibilities

  • Rooms participants for appointments, obtains vital signs, and prepares them for examination and treatment.
  • Performs blood glucose checks, documents results, and promptly reports abnormal findings to the appropriate healthcare provider.
  • Performs medication reconciliation by reviewing and updating medication lists, identifying discrepancies, and communicating concerns to the appropriate healthcare provider.
  • Assists with triage within the scope of LPN practice and established protocols, gathers relevant information, and promptly escalates concerns to the Registered Nurse or healthcare provider.
  • Maintains accurate and timely documentation of participant care, treatments, observations, and communications in the electronic medical record.
  • Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
  • Participates in interdisciplinary team meetings and communicates relevant participant updates to the team.
  • Monitors participant health status and promptly reports changes in condition to the appropriate healthcare provider.
  • Assists with care coordination activities and supports participant care planning and implementation.
  • Participates in utilization review activities, quality improvement projects, and team meetings as assigned.
  • Follows all Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Protects privacy and maintains confidentiality of all company procedures and information about employees, participants, and families.
  • Practices standard precautions and follows Program Infection Control protocols.
  • Performs other duties as required or requested.
