About The Position
Concordia Visiting Nurses is seeking a dedicated and empathetic Licensed Practical Nurse (LPN) to join their growing home health team. As a Home Health LPN, you will be a vital part of the patient's care team, providing skilled nursing care and support in patients' homes. This role offers the opportunity to make a true difference in patients' lives, build deep relationships, and enjoy autonomy and flexibility in managing your own schedule and patient care decisions. The agency provides high-quality skilled nursing, palliative care, and therapy services to patients in nine Western Pennsylvania counties.
Requirements
- A minimum of one year of LPN experience
- Excellent communication, interpersonal, and organizational skills
- Ability to work independently and as part of a collaborative team
- Compassion, empathy, and emotional resilience
Nice To Haves
- Hospice experience is preferred but not required
