About The Position

The Licensed Practical Nurse provides licensed nursing services to residents in accordance with Federal and State regulations and Senior Lifestyle Corporation guidelines. The role supports resident health and safety in a memory care community, including medication administration, care planning, clinical documentation, and communication with residents, families, physicians, and staff. The position may also support resident admissions and discharges, assessments, and clinical tracking under the direction of the director.

Requirements

Training from an accredited licensed or vocational nursing program.
At least 1 year of experience in senior care or a related field.
Current Licensed Practical Nurse license in the state of practice.
CPR certification.
Acceptable proficiency in English to perform job duties, communicate with supervisors and coworkers, and respond to and communicate about emergency situations.
Basic computer skills.
Ability to work as part of a team and handle multiple tasks safely and effectively.
High tolerance for working under pressure and handling critical situations calmly and professionally.

Responsibilities

Administer medications to residents in compliance with Federal and State regulations and Senior Lifestyle Corporation guidelines.
May conduct new-resident admissions, including admission assessments and documentation.
Complete acute and long-term care plans or service plans.
May track clinical matters such as wounds or infection control under the direction of the director.
Provide general consultation on health-related concerns within the community.
Notify physicians and/or family members of changes in residents’ health and provide appropriate documentation.
Respond quickly to emergencies.
Maintain close communication with all departments and staff members.
Provide educational meetings for residents and staff.
Understand the role in the Safety & Disaster Plan.
Maintain confidentiality of residents’ and prospective residents’ information.
May complete discharges, assessments, and clinical documentation as needed.
