About The Position
Navitas Healthcare, LLC is seeking a Licensed Practical Nurse (LPN) – Long Term Care for an exciting Travel or Local job in Grand Junction, CO. This role involves delivering direct nursing care to residents according to care plans, administering medications and treatments as prescribed, monitoring patient conditions and reporting changes to healthcare providers, maintaining accurate nursing documentation and records, and supporting infection control and patient safety standards. The LPN will also collaborate with interdisciplinary healthcare teams.
Requirements
- Active LPN license in Colorado or compact state
- Current BLS/CPR certification required
- Ability to work overnight shifts in a fast-paced environment
Nice To Haves
- LTC or skilled nursing experience preferred
- Strong clinical and communication skills
