About The Position

Valor Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) for a PRN position at the Community Based Outpatient Clinic (CBOC) in Kosciusko. Valor Healthcare is a veteran- and physician-led organization committed to providing high-quality healthcare services to military veterans and federal government employees. The LPN will provide prescribed medical treatment and personal care services to patients under the direction of a physician or mid-level provider. This role involves collaboration with the PACT Team (Primary Care Provider, RN, and Medical Assistant) and expanded PACT Team to develop patient care plans. Requirements

  • Diploma or certificate from an accredited program of practical or vocational nursing approved by the state.
  • Minimum one year of experience as an LPN/LVN in a related primary care or ambulatory care setting; government healthcare environment preferred.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
  • Current certification in Basic Life Support (must be renewed periodically) and in ACLS as specified by individual VAMC contract.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.
  • Energetic and optimistic demeanor.

  • Experience in a government healthcare environment preferred.

Responsibilities

  • Actively assists with patient care, physical examinations, and ancillary tests.
  • Performs annual and routine nursing assessments and preventive health services authorized by the VA and clinic Medical Director, including interviewing, collecting patient information (pain assessment, pulse rate, temperature, height & weight, blood pressure), and recording details in the electronic medical record.
  • Administers medications orally, or by subcutaneous or intramuscular injection, and notes administration details in electronic charts. Applies compresses, observes patients, and reports adverse reactions to the clinic physician.
  • Operates electrocardiograph (EKG) and other diagnostic equipment for routine tests and treatments (glucometers, pulse oximeters, nebulizers, oxygen equipment, spirometry, eye exams).
  • Dresses wounds and incisions, interprets physician's instructions to patients, assists with emergencies, and performs related tasks.
  • Collects lab specimens (urine, blood, sputum) for testing and may perform waived laboratory tests.
  • Cleans treatment rooms, equipment, and supplies using approved materials.
  • Submits laboratory and radiology results to medical providers and scans private practice results into patient records after provider review.
  • Inputs all patient information into VISTA/CPRS.
  • Completes clinical reminders due at the time of each patient visit.
  • May assist with front desk duties for check-in, check-out, and phone coverage.
  • Assists in co-managed care collaboration with non-VA providers and facilities.
  • Provides patient and family health education focusing on self-management, prevention, wellness, related health issues, and healthy lifestyle choices. Advises on community resources.
  • Maintains confidentiality of veterans' data and information.
  • Complies with all VA and company training requirements.
  • Fulfills compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, and lab compliance.
  • Remains focused on achieving excellent clinical outcomes through specified VA guidelines.
  • Provides excellent customer service to veterans, colleagues, and clinic visitors.
  • Establishes and maintains a warm, welcoming, and professional atmosphere.
  • Collaborates with Clinic leadership to resolve veteran concerns or complaints.
  • Participates in clinic outreach events to support enrollment initiatives.
  • Supports patient enrollment and retention in appointments, follow-up visits, or nurse visits.
