About The Position

An exciting opportunity exists for the right individual to join our progressive non-profit organization as an LPN. The position entails working alongside the Assisted Living Program Manager and Care Aids providing care to adults at our Assisted Living locations in Burnaby & Surrey. It may involve some supervision, administration tasks, delegation, providing support and assistance to those who provide care to individuals who are physically disabled with minor mental health issues and/or have acquired brain injuries. The successful applicant would be very organized and would have good leadership skills. Liberty Place (East Burnaby) has 20 – self-contained one-bedroom fully accessible units complete with bedroom, bathroom, living room and kitchen. The building is formed by a community of independent tenants who can direct their own care and require minimal support at key times throughout the day from a team of care aides, hospitality staff, and nurses. Freedom Place (North Surrey) has 20 – self-contained one bedroom fully accessible units complete with bedroom, bathroom, living room and kitchen. Requirements

  • LPN Registry number in good standing and certificate
  • Minimum of 1-year experience

Responsibilities

  • Providing clinical care, support, and assistance to individuals with disabilities
  • Developing and implementing health care plans
  • Completing daily documentation
  • Administration of medication
  • Working as a team
  • Possess knowledge of when to assume assignable and/or delegable tasks
  • Maintain documentation and reports as applicable to each program (e.g. Log Books, Communication Books, Professional Visitation Forms, Quarterly Reports, Care/Service Plans, Individual Program Plans, Reportable Incidents
  • In collaboration with the Program Manager develop tenant care plans and service plans upon entry and when the need arises
  • Update tenants’ progress based on care plan every three months
  • Direct Support Services as well as Administration
