About The Position

The Licensed Practical Nurse (LPN) will be responsible for providing direct patient care in an End-Stage Renal Disease (ESRD) setting. This includes performing patient assessments, administering dialysis therapy, monitoring patient condition, and collaborating with the patient care team. The LPN will also be involved in managing dialysis equipment, ensuring quality assurance, and maintaining patient records. The role requires adherence to company policies, state guidelines, and the Nurse Practice Act, while advocating for patient needs and maintaining a high level of competency in hemodialysis procedures.

Requirements

  • Current license for LPN/LVN in the state of practice.
  • IV certified if state requires.
  • CPR certified.
  • Minimum of one year experience in a hospital or clinical setting.
  • Ability to communicate with all levels, whether professional or non-professional.

Nice To Haves

  • Medical/surgical experience preferred.

Responsibilities

  • Perform patient assessment within 30 minutes of treatment initiation, including vital signs and other parameters for ESRD patients.
  • Review dialysis orders and note general physical and mental condition through observations and questions.
  • Document patient information accurately according to policies and procedures.
  • Provide chronic hemodialysis therapy, including recommending a dialysis plan, initiating dialysis, observing safety measures, and monitoring patient vital signs.
  • Administer medications, fluid therapy, blood products, and other treatments as needed.
  • Develop trust with patients, being sensitive to their fears, feelings, concerns, and body language.
  • Advocate for patient medical intervention for conditions unrelated to renal disease.
  • Practice within the scope of practice for an LPN according to company policies, state guidelines, and the Nurse Practice Act.
  • Participate with the patient care team in developing and reviewing short-term and long-term care plans.
  • Prepare and maintain dialysis machines and systems.
  • Maintain dialyzer records.
  • Prepare dialysis water treatment system and delivery systems for use.
  • Inform technical staff of any equipment or water quality malfunctions.
  • Obtain machine cultures as requested.
  • Ensure changes for dialysis are recorded according to company procedures.
  • Complete all flow sheets.
  • Check stock levels and advise the charge nurse of needed supplies.
  • Participate in quality assurance and cost containment programs.
  • Participate as a member of the patient care team in CQI procedures.
  • Maintain skills and competency related to medications, hemodialysis procedures/machines, and other special equipment.
  • Work overtime with little or no notice.
  • Lift a minimum of 35 pounds and be on feet for long periods of time.
