About The Position

The Licensed Practical Nurse (LPN) is responsible for coordinating, monitoring, and responding to the healthcare needs of program participants. This role provides hands-on nursing care, medication management, health assessments, clinical guidance, and ongoing communication with interdisciplinary team members to ensure individuals receive appropriate and timely care. The LPN serves as an advocate for participant health and wellness while maintaining the highest standards of professionalism, confidentiality, and regulatory compliance.

Requirements

  • Current Licensed Practical Nurse (LPN) License in good standing.
  • High School Diploma or GED equivalent.
  • Valid driver's license.
  • Current vehicle registration and access to reliable transportation.
  • Ability to travel between program locations as needed.

Responsibilities

  • Provide direct nursing care and clinical support to program participants.
  • Assess and respond promptly to health concerns, changes in condition, and medical needs.
  • Administer medications and treatments in accordance with physician orders, nursing standards, and organizational policies.
  • Monitor participant wellness and provide nursing interventions when necessary.
  • Support preventative healthcare initiatives and overall wellness goals.
  • Assist in identifying potential health risks and implementing appropriate interventions.
  • Collaborate with healthcare providers, program staff, and leadership to support quality care.
  • Coordinate healthcare services and communicate participant needs, concerns, and updates.
  • Serve as a clinical resource and advocate for the individuals supported.
  • Participate in health-related meetings, care planning discussions, and team consultations.
  • Promote a person-centered approach to healthcare and support services.
  • Maintain accurate, complete, and timely medical records.
  • Document assessments, treatments, medication administration, and health-related observations.
  • Ensure compliance with healthcare regulations, agency policies, and nursing standards.
  • Monitor adherence to medication protocols and clinical procedures.
  • Protect confidential information and maintain HIPAA compliance at all times.
