About The Position

The Licensed Practical Nurse (LPN) is responsible for providing clinical oversight, coordination, and direct nursing interventions to support the health, safety, and well-being of program participants. This role ensures that all health-related needs are addressed promptly and professionally through assessment, medication administration, direct care services, health monitoring, and collaboration with interdisciplinary team members. The LPN serves as a vital member of the care team by promoting quality healthcare practices, maintaining accurate medical documentation, and ensuring compliance with applicable healthcare regulations and organizational standards.

Requirements

  • Current and valid Licensed Practical Nurse (LPN) license in good standing.
  • High School Diploma or equivalent.
  • Valid driver's license, current vehicle registration, and access to a reliable, functional vehicle.
  • Knowledge of Bloodborne Pathogens, Right-to-Know regulations, HIPAA, Security Acts, and Fire & Safety procedures.
  • Successful completion of all required training programs and certifications.
  • Strong clinical nursing knowledge and practical application of nursing principles.
  • Proficiency in medication administration and medication safety practices.
  • Excellent assessment, observation, and critical thinking skills.
  • Attention to detail and accuracy in recordkeeping and documentation.
  • Effective verbal and written communication skills.
  • Ability to respond calmly and appropriately in crisis situations.
  • Strong organizational and time-management abilities.
  • Collaborative approach to team-based care.
  • Commitment to ethical practice, professionalism, and confidentiality.

Nice To Haves

  • Experience providing nursing care in community-based, behavioral health, developmental disability, residential, or human services settings preferred.
  • Experience with medication administration, healthcare documentation, and care coordination.

Responsibilities

  • Provide hands-on nursing care and clinical services to program participants.
  • Assess participant health needs and respond promptly to medical concerns and emergencies.
  • Administer medications, treatments, and other prescribed healthcare interventions in accordance with physician orders and established protocols.
  • Monitor participant health status and report changes or concerns to appropriate healthcare providers and leadership.
  • Deliver health-related interventions that promote wellness, safety, and quality of life.
  • Participate in crisis response and emergency situations requiring nursing expertise.
  • Collaborate with program staff, healthcare providers, family members, and community resources to ensure continuity of care.
  • Coordinate medical appointments, follow-up care, and treatment recommendations as needed.
  • Communicate participant health updates and concerns to supervisors and relevant team members.
  • Support implementation of individualized care plans and health-related goals.
  • Maintain accurate, timely, and confidential medical records and documentation.
  • Ensure compliance with agency policies, healthcare regulations, and nursing standards of practice.
  • Monitor adherence to medication administration procedures and medical protocols.
  • Prepare incident reports and other required clinical documentation.
  • Safeguard protected health information and maintain strict confidentiality in accordance with HIPAA requirements.
  • Follow all OSHA, HIPAA, Fire Safety, and organizational policies and procedures.
  • Attend and actively participate in required trainings, meetings, and professional development activities.
  • Exercise sound clinical judgment and maintain a professional demeanor at all times.
  • Support a collaborative, team-oriented work environment.
  • Perform additional duties as assigned to support effective program operations.
