About The Position

A Licensed Practical Nurse (LPN) is a trained healthcare worker who, under the direction and supervision of a Registered Nurse (RN), provides nursing care to residents. The LPN assists the RN in caring for acutely and chronically ill residents. The work performed is assisting and technical in nature and is regulated by the organization's policies and procedures. The judgment required is based upon a minimum of scientific knowledge and is limited to activities that are largely repetitive and routine.

Requirements

Graduated from a state-approved school of Practical Nursing.
Passed the State Board Examination.
Licensed to practice as a Licensed Practical Nurse in the State of Georgia.
Active American Heart Association BLS certification within 30 days of hire.
Ability to communicate in English, both verbally and in writing.
Basic Computer Skills.

Responsibilities

Plans and organizes resident care under the guidance of the Assistant Director of Nursing (ADON)/Director of Nursing (DON).
Fills out work assignments from the resident activities record at the beginning of the work tour.
Provides for elimination needs, including bedpans, enemas, catheterization, and insertion of Foley catheters.
Collects specimens such as urine, stool, and sputum, and cultures.
Prepares residents for meals, assists with serving and feeding, and administers gastric feedings. (Note: LPNs do not insert gastric tubes unless they have had special training).
Measures and records intake and output.
Takes and records temperature, pulse, respiration, and blood pressure, reporting any deviations from the normal to the RN.
Monitors intravenous infusions and removes needles as directed by the RN, applying Band-Aids.
Observes and accurately records the results of treatments and nursing care on nursing records.
Reports observable changes in residents' condition and other pertinent information to the RN.
Works with the RN in the care of residents requiring professional nursing skills.
Carries out the part of the nursing care plan delegated by the RN.
Administers irrigations.
Cares for residents receiving oxygen by mask, catheter, or cannula.
Performs nose and throat suction using bulb syringes or a suction machine.
Performs simple dressings and assists with complicated dressings.
Provides first aid.
Positions residents under direct orders from the RN.
Administers medications as assigned by the RN.
Contacts attending physicians regarding changes in resident’s condition.
Completes informative and accurate summaries monthly for skilled residents (daily) and ICF residents (every two weeks).
Charts results on all PRN medications.
Completes outpatient forms, infection control forms, Lab forms, and X-ray requisitions as needed.
Provides adjustments in environmental factors such as lighting, ventilation, cleanliness, and order of the bedside unit and equipment.
Gives informative shift reports to the oncoming shift.
Passes medications per the 5 rights of administration.
Completes all occurrence reports in a timely manner and turns them into the ADON.
Consistently puts new orders in Matrix in a timely manner.
Consistently completes documentation for behavioral changes in the chart.
Completes weekly skin assessments accurately and provides a copy of the WCN.
Completes behavioral forms every shift.
Updates nursing and CNA care plans as needed.
Completes daily documentation for MAR, TAR, falls, med A, new/readmits.
Initiates new event charting in the chart for concerns/new findings with residents.
Reviews the message board throughout each shift, addresses as appropriate, and archives messages.
