We are currently hiring for a LPN with acute care experience to w ork 36 hours per week Day/Evening Shift, to support the Medical Specialty Unit (MSU), with holidays and weekend per the guidelines of the department.

Requirements

Graduate of an accredited school of practical nursing
Acute Care experience preferred
Per offer of employment letter, signed commitment upon hire to obtain RN license and then subsequent BSN degree within the defined time frame.

Responsibilities

Conducts ongoing patient observations to gather data relevant to the patient needs.
Records conditions and patient’s response to treatment.
Notifies assigned/team RN of any changes in condition.
Collaborates and assists RN in the development, implementation, and evaluation of the nursing plan of care for the patient.
Updates the plan as appropriate.
Performs assigned nursing procedures for comfort and well-being of patients demonstrating a concern for patient safety.
Administers medications in a safe and effective manner.
Participates in patient/family education and discharge planning, documenting appropriately.
Updates the teaching plan as appropriate.
Establishes priorities appropriately, based on patient needs.
Participates in evaluating the quality and effectiveness of nursing practice.
Demonstrates skills and knowledge necessary to provide care to adult and geriatric patients including knowledge of growth and development, the ability to obtain information and to provide the care needed.
Communicates effectively to patients, families, and the health care team, utilizing both written and verbal communication skills.
Responds to patient crisis situations as outlined in unit specific standards.
Performs assigned work safely, adhering to established CCHS and departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients, and visitors.
Completes mandatory in-service education.
Performs other related duties as required.
