About The Position

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a Long-Term Care (LTC) setting. The LPN will work under the supervision of a Registered Nurse or healthcare provider to support resident health, safety, and overall well-being.

Requirements

• Active New York State LPN License
• BLS Certification (AHA)
• Minimum 1 year of LPN experience
• Long-Term Care Nursing
• Medication Administration
• Resident Assessment and Monitoring
• Treatment Administration
• Clinical Documentation
• Patient and Family Communication
• Infection Prevention and Control
• Care Plan Implementation
• Interdisciplinary Team Collaboration

Responsibilities

• Provide direct nursing care to long-term care residents
• Administer medications and treatments according to physician orders and facility protocols
• Monitor resident conditions and report changes to the healthcare team
• Assist with implementation of individualized care plans
• Document assessments, treatments, and resident responses accurately
• Collaborate with RNs, providers, CNAs, therapists, and other healthcare professionals
• Support residents' daily health and wellness needs
• Ensure compliance with facility policies, safety standards, and regulatory requirements
