About The Position

The Licensed Practical Nurse (LPN) is responsible for participating in assessing, implementing, and documenting resident care as ordered by a physician, with the continuous availability of a registered nurse. The Sharon at SouthPark is a continuing care retirement community for senior residents, offering independent living, assisted living, and skilled nursing care. Requirements

  • Ability to triage and provide skilled nursing care to residents and employees.
  • Ability to communicate effectively; orally and in writing.
  • Ability to work with necessary medical equipment and perform required tests and treatments as ordered by a physician, per facility policies and procedures.
  • Ability to function effectively within a team environment.
  • Ability to deal effectively with confused and/or agitated residents.
  • High School Diploma required.
  • Graduate of an accredited school of nursing.
  • Current unencumbered North Carolina Licensed Practical Nurse license required.
  • Ability to identify special resident problems and report them immediately to supervising nurse.
  • Ability to identify safety hazards and emergency situations, initiate corrective action as necessary and/or seek assistance of supervising nurse.
  • Recognition that errors in judgment may jeopardize the safety of the residents.
  • Proof of Influenza vaccine and/or approved medical/religious exemption is required for all employees.

Responsibilities

  • Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic, and rehabilitative nursing care of assigned patients.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Perform triaging via telephone and in-person, of residents and employees, taking further action as findings indicate.
  • Handle emergency situations in a prompt, precise, professional, and safe manner, determining appropriate interventions and notifying relevant parties.
  • Complete incident reports and applicable follow-up forms accurately and timely.
  • Prepare and administer medications, tests, and treatments accurately, observing patient response, and documenting outcomes.
  • Participate in ongoing patient assessment and evaluate patient responses to nursing actions under the guidance of a registered nurse.
  • Document changes in patient conditions and notify physician, responsible party, and supervising RN of changes.
  • Complete appropriate documentation and other record-keeping duties in patient's electronic medical records (Point Click Care).
  • Communicate with internal and external physicians regarding patient care and condition.
  • Transcribe, clarify, coordinate, and implement physician orders.
  • Communicate with residents, physicians, family members, and other members of the health care team.
  • Comply with company policies, procedures, and related local, state, and federal regulations.
  • Maintain accountability for controlled substances and inventories medications at the beginning and end of each shift with another licensed nurse.
  • Support the facility's medical supply system and follow procedures for patient charge items.
  • Follow clinic fee schedule with regards to patient charges for time and services rendered.
  • Remain current on nursing techniques and advances by attending in-services, workshops, seminars, and continuing education opportunities.
  • Ensure supplies and equipment necessary for patient care are stored in an organized, safe, and efficient manner.
  • Communicate and coordinate resident medication orders with Pharmacy as needed.
