Optimal Hospice is looking for a Licensed Practical Nurse (LPN) to join their team. As a Licensed Practical Nurse (LPN) at Optimal Hospice, you will play a vital role in delivering high-quality care to our patients. Working closely with our interdisciplinary team, you will provide compassionate support to individuals and their families during their end-of-life journey. Requirements
- Has completed a practical (vocational) nursing program and is licensed in good standing in the state of Colorado and who furnishes services under the supervision of a qualified Registered Nurse.
- Must be able to communicate in the language of the client and document client related activity as appropriate
- Possesses adequate and effective written and verbal communication skills.
- Possess adequate and effective interpersonal skills.
Nice To Haves
- One (1) year of experience as a Licensed Practical Nurse with Hospice experience.
Responsibilities
- Provide direct patient care under the supervision of a registered nurse.
- Conduct assessments, monitor patient conditions, and communicate findings to the healthcare team.
- Assist with personal care activities if needed.
- Offer emotional support and companionship to patients and their families.
- Collaborate with other members of the interdisciplinary team to develop and implement individualized care plans.
- Maintain accurate and up-to-date documentation of patient care activities.
- Records clinical/progress notes in client/patient's clinical record.
