About The Position

Affirmed Home Care is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide one-on-one care for a patient in Chester, NJ. This private-duty case requires strong clinical experience, professionalism, and a proactive approach. The patient is bedbound and requires attentive, hands-on nursing care, including medication management (reminders). The home is equipped with a Hoyer Lift, Hospital Bed, and PureWick Catheter. Respiratory therapy is currently in place, with additional services being coordinated.

Requirements

• Active NJ LPN license
• Experience with trach/vent cases strongly preferred
• Professional, reliable, and proactive
• Ability to provide safe, high-quality care in a home setting

Responsibilities

• Provide one-on-one care for a patient in Chester, NJ.
• Perform Trach care.
• Manage Ventilator support.
• Administer Cough assist.
• Perform Suctioning.
• Manage feeding tubes.
• Provide PureWick catheter care.
• Assist with Hoyer lift transfers.
• Provide medication management (reminders).
