About The Position

Make a difference every day at Lakeview Health Center. We are seeking a dedicated Licensed Practical Nurse to join our Long-Term Care team. This position works under the supervision of a Registered Nurse, providing high‑quality, person‑centered care to residents in a collaborative and supportive environment.

Requirements

• Graduate of an accredited nursing program
• Current Wisconsin LPN license (required)
• Valid WI driver’s license and acceptable driving record (if driving required)
• Successful completion of background check per Wisconsin Caregiver Law

Nice To Haves

• Experience in long-term care preferred

Responsibilities

• Provide nursing care within the provisions of the Nurse Practice Act
• Administer medications and treatments under RN supervision
• Document clinical observations accurately in resident records
• Assist in training and orientation of new staff
• Support development and implementation of resident care plans
• Maintain resident rights, confidentiality, and safety standards
• Prepare daily shift reports and communicate resident status changes
• Ensure efficient use and supervision of Nursing Assistants
• Participate in in‑service education and ongoing training
• Promote person‑centered care and uphold a positive, respectful environment
