About The Position

Pinnacle Home Care is hiring a Licensed Practical Nurse (LPN) to provide compassionate home health care in New Smyrna Beach and surrounding areas. The role delivers individualized care in patients’ homes and supports healing, independence, and positive patient outcomes. The nurse administers skilled nursing care under RN supervision and completes required documentation in the electronic medical record. The position involves visiting patients’ homes, some of which may not be wheelchair accessible, and performing physical care activities.

Requirements

• Current state-specific LPN or LVN license.
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least one year of nursing experience.
• Excellent communication, organizational, and time management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
• Ability to walk, stand, stoop, and occasionally lift or turn patients.

Nice To Haves

• Home health or medical/surgical nursing experience preferred.

Responsibilities

• Administer skilled nursing care under RN supervision.
• Perform curative, rehabilitative, or preventive care tasks outlined in patient care plans.
• Apply clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes.
• Complete visit reports and required documentation promptly and accurately in the electronic medical record system.
• Confirm daily visit schedules to coordinate care with other staff.
• Demonstrate awareness of and participate as required in the agency’s Quality Assurance and Performance Improvement (QAPI) program.
• Follow the agency’s process for managing and addressing customer complaints.
• Uphold patient rights in accordance with agency service agreements and admission policies.
• Understand and follow agency policies regarding appropriate patient discharge and reasons for discharge.
