About The Position

As a member of the skilled nursing team, the Licensed Practical Nurse, under the direction and supervision of the Registered Nurse, is responsible for giving resident care using the nursing process and ensuring residents receive care outlined in the nursing care plan. Services to be provided for residents over 16 years of age regardless of age, race, religion, and cultural background. Majority of services provided for the elderly.

Requirements

LPN - must be licensed in the state of practice.
Maintains CPR certification.
Ability to read, write, and understand written and oral communication.
Follows universal precautions and utilizes proper body mechanics.
Keeps up to date in the field of nursing, especially geriatrics.
Act 34 clearance (criminal background check).

Experience is preferable but not required.

Responsibilities

Gives resident care using the nursing process and ensuring residents receive care outlined in the nursing care plan.
Provides direct resident care.
Assists with resident and family teaching.
Provides input into resident care plans.
Maintains residents’ rights as established.
Supervises nursing assistants.
Makes rounds of residents, observes and evaluates resident’s needs, condition, and documents care given.
Keeps RN/supervisor informed of activities on unit.
Assists in orienting new personnel.
Meets with the Director of Nursing to plan unit work routines, implementation of policies/procedures and to discuss unit needs/problem solving.
Keeps informed of fire, safety, and disaster programs.
Attends designated in-service programs.
Interacts and directs staff in a positive and professional manner.
Attends staff meetings.
Participates in Performance Improvement activities.
Participates in team conferences.
Other duties as assigned.
