Under the supervision of and in collaboration with the RN, performs responsibilities exercising initiative, judgment and independent decision-making within the scope of the State Nurse Practice Act, hospital policy and other governing organizations. Demonstrates adaptability and flexibility to changing situations including cares, medication and behaviors. Identifies changes and trends in resident status. Individualizes care plans for residents depending on needs. Monitors resident outcomes and effectiveness of plan of care. Documents all findings. Practices safety and infection control measures. Completes treatments, med administration and equipment use following established standards. Responds appropriately to emergency situations. Observe for resident/family learning needs. Plans and coordinates with other disciplines (P.T., Pharmacist, etc.) to meet learning needs of residents. Teaches, counsels and reinforces residents and family members. Assumes accountability and responsibility for quality of resident care provided. Demonstrates initiative and good judgment in all situations. Fosters support and team building with residents & families, RCA's, and manager. Ordering nursing supplies.
