Job Title: Licensed Practical Nurse (LPN) – Private Duty Nursing-Shared Case Service Location: Middleburg, PA and surrounding areas Shift: Weekday (Full-time and Part-time schedules available) Rate: Base Rate up to $31 plus a Shared Premium Rate Immediate Need: Day Shift hours (Monday through Friday) Home is a smoking household Traditional Home Health Care (Pediatric & Adult Skilled) is a leader in the healthcare field, providing exceptional skilled nursing and aide services. Responsibilities
- One to One patient care in home/school environment
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members
