About The Position

St. Elizabeth Home, an Illuminus Community, is hiring a Licensed Practical Nurse to provide prescribed medical treatment under the general supervision of a registered nurse. The role is based in a skilled nursing setting that provides long-term care and short-term rehabilitation. The position may include 8-hour shifts and weekend rotation as scheduled; shift availability may vary based on community needs. Full-time, part-time, and PRN scheduling opportunities are offered. Pay is $27.80 to $31.20 per hour, dependent upon experience. The workplace is at 109 S Atwood Ave, Janesville, WI 53545.

Requirements

• Active Licensed Practical Nurse credentials and a license in the State of Wisconsin in good standing.
• Active CPR.
• Knowledge of and/or experience in geriatrics.

Responsibilities

• Maintain effective, professional communication with residents, patients, clients, families or personal representatives, and staff.
• Promote a positive organizational image consistent with the mission, vision, and values; serve as a professional role model and provide on-call services as assigned.
• Develop, implement, and update plans of care.
• Promote resident-centered care and support residents in achieving and maintaining their highest practicable level of health and well-being.
• Administer and monitor medications, perform medical treatments and vital sign checks, and verify physician orders for accuracy and appropriateness.
• Lead, guide, and supervise CNAs and resident assistants in providing quality care.
• Assist with physician visits, provide staff feedback, and ensure appropriate delegation, direction, and follow-through of assigned duties.
• Observe, document, and report resident status, including changes in condition and 24-hour reports.
• Promptly communicate concerns to the registered or charge nurse, family, or physician as appropriate.
• Provide thorough transfer of information to the next team leader.
• Follow universal precautions and PPE requirements.
• Perform other duties as assigned.
