About The Position

Haven Home Care, a trusted provider of quality home healthcare services since 1998, is expanding its proven care model to Eastern Ohio, including Ashtabula, Columbiana, Mahoning, and Trumbull counties. This is a per-visit role, not an hourly job, offering supplemental income. The position involves completing open visits, collecting documentation, conducting supervisory visits for Home Health Aides every 60 days, and completing pre-employment testing for aide candidates. This role is ideal for someone currently employed elsewhere or with flexible schedule availability, seeking additional income, and comfortable with building a territory from zero clients. Initial hours will be minimal, potentially 5-10 hours per week or less, and will grow as the client base expands.

Requirements

Must be at least 18 years old
Graduation from an accredited School of Practical Nursing
Current Ohio LPN license in good standing
Valid driver's license, reliable transportation, and current auto insurance
Current CPR certification
Successful completion of criminal background check, skills test, and written nursing test
One or more years in a home health agency or in a hospital setting.
Residence in Trumbull or Mahoning County preferred

Nice To Haves

Home health experience preferred.
Experience with scheduling, documentation, and compliance regulations not required but a plus.

Responsibilities

Assist in carrying out personalized care plans as directed.
Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
Conduct skills tests and written tests for Home Health Aide candidates.
Foster cooperative relationships among the healthcare team by understanding each person's role and actively participating in case conferences.
Assume responsibility for ongoing professional growth through participation in nursing organizations and in-service programs.
