About The Position

Haven Home Care is expanding its services to Eastern Ohio, including Ashtabula, Columbiana, Mahoning, and Trumbull counties. This is a per-visit role, not an hourly position, offering supplemental income. The role involves establishing the agency's presence in a new territory, with initial hours expected to be minimal and grow as the client base expands. The Licensed Practical Nurse will be responsible for completing open visits, collecting documentation, conducting supervisory visits for Home Health Aides, and performing pre-employment testing for aide candidates. This position does not involve skilled nursing care or medical assessments. The ideal candidate is currently employed elsewhere, seeks additional income, is comfortable with variable hours and building a territory from scratch, possesses an entrepreneurial mindset, and is patient and flexible.

Requirements

Must be at least 18 years old
Graduation from an accredited School of Practical Nursing
Current Ohio LPN license in good standing
Valid driver's license, reliable transportation, and current auto insurance
Current CPR certification
Successful completion of criminal background check, skills test, and written nursing test
One or more years in a home health agency or in a hospital setting.
Excellent documentation and record-keeping abilities
Effective time management and organizational skills
Communication skills with ability to interact professionally with clients, families, and healthcare team
Knowledge of home health regulations and practices
Ability to work independently and schedule visits efficiently
Compassionate and patient-centered approach
Reliability and strong work ethic
Adaptability to different home environments and patient situations
Problem-solving mindset
Professional demeanor and boundary awareness
Proficiency in utilizing calculator, multi-line telephone
Competent operation of company-provided iPad with data plan
Appropriate use of company-provided name badge and cardigan with "Haven" logo

Nice To Haves

Home health experience preferred.
Experience with scheduling, documentation, and compliance regulations not required but a plus.
Residence in Trumbull or Mahoning County preferred.

Responsibilities

Assist in carrying out personalized care plans as directed.
Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
Conduct skills tests and written tests for Home Health Aide candidates.
Foster cooperative relationships among the healthcare team by understanding each person's role and actively participating in case conferences.
Assume responsibility for ongoing professional growth through participation in nursing organizations and in-service programs.
