About The Position

The Licensed Practical Nurse (LPN) is a member of the PACE interdisciplinary team; provides nursing care to the participant and assists the Registered Nurse with the implementation of the Plan of Care. The LPN provides care for the participant in a manner that promotes an interdisciplinary team approach, respects participant’s goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU PACE. Participates in all areas of clinical services.

Requirements

  • Must possess a current LPN license in the state of Colorado
  • Minimum one year LPN experience
  • Must successfully pass agency required background checks and drug screening
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100K Property Damage coverage) and have reliable transportation
  • Requires travel to multiple work locations
  • Must possess CPR certification or be able to obtain within 60 days of hire
  • Must comply with agency health screening and immunization requirements
  • Must be able to demonstrate computer competency

Nice To Haves

  • hospice or home health experience preferred
  • Experience working with frail/elderly preferred
  • Experience working with individuals with mental health needs desirable
  • Bilingual in Spanish preferred
  • IV certification and phlebotomy skills preferred

Responsibilities

  • Collects data regarding all body systems, signs and symptoms, deviations, acuity, vital signs, and responses in collaboration with the Registered Nurse and primary care physician,
  • Incorporates participant preferences and goals within the plan of care
  • Supports and respects the participant and caregiver’s unique diversity, including spiritual and cultural beliefs.
  • Completes documentation in an accurate and timely manner as per agency standard and in compliance with regulatory standards
  • Provides participant and caregiver education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, and health and wellness through the lifespan
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely open manner; engages collaboratively in conflict resolution processes as needed
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external
  • Participates in mandatory educational activities and required meetings
  • Is competent using agency information and technology systems
  • May assist Physician or Clinic RN in procedures and assist with participant / staff immunizations as needed
  • Will participate in on-call rotations including nights, weekends and/or holidays
  • Will act only within the scope of his / her authority to practice
  • Will include participant care in all settings
  • Performs focused initial and focused reassessments
